Academic Success Program Form
To be filed after chapter has attended/hosted an Academic Success/Tool Program, within 2 weeks fo program. Sorority requirment only. 

Alcohol Program Form
To be filed after chapter has attended/hosted an Alcohol Awareness Program, within 2 weeks of program.

 Greek Service Reporting Form
To be filed after chapter completes any community service

 Chapter Advisor Roster Form
Due at the first council meeting of each semester

Chapter Financial Information Form
Due July 1st

Chapter Officer Roster Form
Due one week after chapter elections

Continuous Open Recruitment Agreement
Due within 24 hours of the extension of the bid
